New Clients
If you are new to the clinic and have not yet filled out a new patient form please find one attached to the email. If you would like to send back to us, prior to your appointment that is not a problem the email address is info@align.je or you can print out and bring along with you.
If you do not wish to complete the form before your appointment please make sure you allocate some time before your appointment to complete.

Cancellation Notice
Please note if you need to reschedule or cancel your appointment no less than 24 hours notice is required to avoid any charges, and for further details we can provide you with our clinic policy.
No Show or on the Day Cancellation (less than 12 hours notice) will be charged at the full session fee. If you cancel within 12-24 hours a half price charge will be taken.

We ask all clients to settle their accounts following their sessions including any charges for missed appointments. We reserve the right to charge an administration fee of £5 to any accounts that are not paid directly after the session. We thank you in advance for your cooperation with this matter.
